Travel Advice
Please make an appointment at least eight weeks in advance of your holiday to ensure adequate cover. A charge will be made for certain immunisations and vaccinations which are not covered by the NHS. A list of these charges is held in reception.
Patients are now required to complete a travel risk assessment form before you can see the nurse to get any vaccinations. If this form is not completed before your appointment, this will result in you not being seen. The travel risk assessment forms are available from reception or alternatively you can download and print the form by clicking the link below. Further travel information will shortly be available to download.
