How To Register
How To Register With The Practice
The guidelines for registering have now changed and the following will be required.
All new patients wishing to register must check whether they fall within the catchment area of the practice. Proof of your address (utility bill/ bank statement) and a form of ID (passport/ driving licence) is required to register. If you do have your previous NHS card please bring this with you as it enables us to locate and retrieve you medical records as quickly as possible.
For children aged 5 or under, it is a requirement that parents must provide a copy of their child's birth certificate, their NHS number and their red books so the surgery can have up-to-date record of your child's vaccinations before we can register them.
All new patients aged 16 and over must have a new patient health check as this is part of the registration process. If you change your address or telephone number please inform the surgery as soon as possible.
